What types of insurances do you accept?

We are currently in-network and accept these insurances:

  • Aetna (Medicare and Medicaid)
    BCBS Community Health Plan
    BCBS- PPO & HMO
    Humana
    Meridian
    Molina
    United Health Care

    Medicaid & Medicare

We are currently working on getting in-network with Cigna and CountyCare, but we do not refuse service to those with these insurances.

Do you accept self-pay patients?

Yes, we accept self-pay patients and provide a sliding fee scale for those who do not currently have insurance or underinsured. The sliding fee scale is based on monthly income and number of people in the household.

What types of Medication Assisted Treatments/Recovery (MAT/MAR) services do you provide?

Our providers safely prescribe Suboxone, Subutex, and Sublocade Injections, for more information visit Our Services page. They also use contingency contracts for harm reduction.

What makes you different from other providers/clinics?

Our main focus is to provide care for those with substance use disorder (SUD) and to be part of the solution to the opioid crisis that is going on in the world is our mission. Our clinic is located in the area with the highest overdoses and deaths causes by opioids and we have the opportunity to lower those numbers by providing care in this “hot spot” and bring in those patients with SUD.

What is contingency contracts/management?

Contingency contracts/management is essentially a change reward. You set up a goal that is S.M.A.R.T, specific, measurable, achievable, relevant, and time-bound. For example, a SMART goal for weight loss can be, I want to lose 5 pounds in 3 months by exercising 3 times a week. BaxZease, LLC is the contract management company we are partnered with.
